Please note that in order to get appointed with us, you must have a minimum agency premium of $500,000, and must comply with our yearly minimum volume commitment of $50,000 for our Work Comp & P&C Departments, and/or $25,000 for our Professional Liability and High Value Homeowners’ Departments.

**Until your agency is appointed with us, we cannot quote or bind any submissions.**

If your agency meets our appointment requirements, then please complete and return the following items, and we will be in contact with you shortly to advise you on the status of your appointment:

• AWIS Producer Questionnaire (must be filled out in entirety);
• Signed AWIS Producer Agreement;
• Completed W-9;
• Copy of your current Errors & Omissions Declarations page;
• Copy(ies) of current licenses for the states in which you write business;
• Copy of your Brokers Bond (CA only)
